Documents Required for FSSAI Registration/License:

1. Identity and Address Proof:

   • PAN Card of the applicant

   • Aadhaar Card/ Voter ID/ Passport/ Driving License of the proprietor/partners/directors

   • Passport-sized photograph

2. Business Proof:

   • Certificate of incorporation (for companies)

   • Partnership deed (for partnerships)

   • Articles of association or memorandum of association (for companies)

3. Premises Proof:

   • Rental agreement/Ownership document

   • Electricity bill or property tax receipt

4. Food Safety Management Plan (FSMP):

   • Description of the food business activities

   • List of food products to be dealt with

5. Additional Documents:

   • NOC from the municipality or Panchayat (for food businesses operating in premises not owned by the applicant)

   • Copy of the certificate obtained under the Co-operative Act, 1861/Multi-State Co-operative Act, 2002 (if applicable)
